Leeds had further chances to score with Kemar Roofe and Hernandez again going close before half time.
David Cotterill proved City's main threat going forward but the visitors were never quite in the game and had Tammy Abraham come off injured for Matty Taylor before the break.
Substitute Milan Djuric twice spurned chances to get City back in it in the second half before finally firing the ball past Rob Green.
But it was too late as the Bosnian's effort came in the seventh minute of stoppage time.
The win keeps Leeds in fifth place in the Championship as Huddersfield, Reading and Sheffield Wednesday all won their fixtures, while the top two – Newcastle and Brighton – drew against Norwich and Ipswich respectively.
